The form can be completed electronically. Please attach each form individually if emailing more than one form for families.
Please note, Dr. Khojasteh has limited spots available and may not be able to accept all patients who join the waitlist. Acceptance is on a first come first serve basis.

Please complete and return this form via fax, e-mail or in person at the clinic. Once we have received your form, our staff will contact you within 1 to 2 business days to schedule your first appointment.
Note: If you are sending completed registration forms via email for more than one person, please attach each person’s registration form as a separate file to the email.
When you arrive for your first appointment, please remember to bring your Ontario Health Card and a list of all medications you are currently taking, including vitamins and over the counter medications.
